Youth & Family Services

Introduction

Our department of children, youth, and family provides inclusive services for children and youth starting at age 5. It is through this department that we look to empower the families we serve as a whole, in order to strengthen our community.
Our support services include a Mental Health Consultant who conducts observations to determine effective strategies and resources for special needs children. We have a case management approach and services are designed to minimize disruption in the flow of the daily schedule. Each child has an individualized education plan in order to reach their full potential.

Day Habilitation Services

  • Medicaid Card, Birth Certificate, Social Security Card, State ID or Passport, Proof Of Income (SSI or SSA), Psychological and Psychosocial Evaluations, Medical Exam.
