Documents needed for enrollment appointment
All Parents
Birth certificates and social security cards of all children in household under age 18 and 2 proof of address document
In addition, the following documents are needed:
Foster Parents
Letter from foster care agency
Working Parents
6 Current pay-stubs (if paid every 2 weeks) or 12 pay-stubs (if paid every week or if your hours change every week). Pick up a salary verification form (must obtain from center's office) if you do not have enough pay-stubs or if you get paid in cash.
Public Assistance Recipients
Budget letter from HRA stating amount received (HRA may require you to submit form W474 to center. Please inquire.)
Homeless Residents:
Letter from shelter or notarized letter from person you are residing with.
Going to school:
Pick up a vocational form to take to school or college.
If you or your children were born outside the U.S.:
You must bring a permanent resident card and passports for parent and child / children.
